- Must complete the classroom portion
- What do you need to take the permit test
- Parent, guardian or sponsor
- Must take social Security card
- Birth certificate
- mv3001 form sign by us
- PLEASE NOTE- If the course is completed on a weekend-The permit form (MV3001) will NOT be issued until the next business day!
